Health insurance can frequently be complicated or confusing, and Premier Family Health & Wellness strives to do our best to assist our patients with any questions they may have.
We accept most major health insurance plans including, but not limited to:

  • Anthem (formerly Wellpoint)
  • AvMed
  • Cigna
  • Coventry
  • Blue Cross Blue Shield (both Florida Blue and out-of-state)
  • GHI
  • Great West
  • Highmark
  • Humana
  • Medicare
  • Neighborhood Health Plan
  • Oxford
  • Tricare
  • United Medical Resources (UMR)
  • United Healthcare
The following insurance plans are only accepted in our Premier Urgent Care Center
  • Blue Cross Blue Shield’s “My Blue” Plan
  • Personal Injury Protection (PIP)
  • Worker’s Comp
Premier also accepts insurance plans that are connected via PHCS MultiPlan and many forms of student health plans offered to Palm Beach County schools, such as Florida Atlantic University and Lynn University.
Please remember that patients with an HMO health insurance plan must have Premier Family Health or Dr. Vincent Apicella, DO listed as their Primary Care Provider (PCP). If a patient’s insurance card has another provider’s office listed, we will not be able to bill their insurance company for the visit.
Patients who switch insurance companies should notify us as soon as their plan takes effect.

  • Lifetime Health Medical Group
  • Medicaid and/or all related Medicaid plans
  • Metropolitain
  • Molina Healthcare
  • Travelers Health Insurance
  • WellCare Health Plans
